dc.description.abstract Introduction: The World Health Organization established health system responsiveness in 2000 to address non-clinical components of health service quality, which are assessed using eight domains. Despite the aim for a responsive health system in low and middle-income countries especially in our country, there is limited evidence on the status of delivery care health systems‟ responsiveness. Objective: To assess the health system responsiveness and its associated factors in delivery care at West Arsi Zone public hospitals, Oromia, Ethiopia, 2022 G.C. Methods: A hospital-based cross-sectional study was conducted in all public hospitals in the West Arsi zone from March 28 to April 26, 2022. A systematic random sampling technique was used to select 574 study participants. A semi-structured interviewer-administered questionnaire was used to collect data. The data were entered into EpiData version 3.1 and analysed by using a statistical package for social sciences version 26. Bivariate and multivariable logistic regression analyses were used for identifying the factor of health system responsiveness and which have statistically significant association were identified based on p-value ≤ 0.05 and adjusted odds ratio, with a 95% confidence interval in multivariable logistic regression. Finally the result presented with graph and tables. Result: The finding of this study showed that 47.5% of the mothers got good health system responsiveness in delivery care. Women who living in urban (AOR=0.58, 95%, CI: 0.4- 0.85)(P=0.005), family monthly income ≤650 ETB(AOR=1.78%, CI: 1.03-3.07)(P=0.038), and presence of obstetric complication(AOR: 0.62, 95% CI: 0.4-0.98)(P=0.04) were factors associated with good health system responsiveness Conclusion: Due to the lower health system responsiveness score in the delivery care the study area than the overall Ethiopian score, needs more responsive health system; stakeholders should emphasize on taking the amendments to the hospital's environment, service and care; updating health care providers on responsiveness domains to improve the score en_US
