Abstract
Background: A woman‟s satisfaction with the delivery service may have immediate and long-term effects on her health and successive utilization of the services. Mothers‟ delivery care satisfaction is one of the indicators to monitor the quality of health care provision. However, there is studies in this regard in Ethiopia, there were limited studies that assess maternal satisfaction particularly in the study area. Therefore, this study aimed to assess delivery service satisfaction and its associated factors under free healthcare policy among mothers who gave birth at public health facility of Gamo Zone.

Methods: facility based cross sectional study was conducted. A total of 384 mother who give birth in selected facilities were enrolled in the study. The study participants were selected by systematic random sampling and interviewed using structured questionnaire adopted from different literatures and contextualized to the local situations and study objectives. Logistic regression model was used to identify the association between explanatory and outcome variables. Adjusted Odds Ratio with 95% CI was used to measure the strength of the association between these variables. A p-value < 0.25 at bivariate analysis was considered for variables to be candidates for multivariable logistic regression analysis. Variables with a p-value of < 0.05 at multivariable analysis were considered as statistically significant factor of mothers‟ satisfaction.
Result: A total of 374 mothers who give birth at public health facility were participated in the exit interview with 97.4% response rate. The overall proportion of mothers who were satisfied with the services was 78.9%(95% CI: 74.4 – 82.7%). The odds of reporting satisfaction were five times higher among mother who planned for pregnancy, as compared with mother who had unplanned pregnancy (AOR = 5.52; 95% CI;(1.96-31.641). Mother who pay for any of the services experiences a reduction of 65% in the odds of satisfaction compared to mother who had free service (AOR =0.347; 95% CI;(0.151-0.796). Willing to recommend the facility for future health care need were three times more likely to be satisfied with the delivery service when compared to the counterpart AOR = 3.201 95%; CI;(1.160-8.830). The odds of satisfaction were 3.8 times higher among mother who give birth in Primary Hospitals than General Hospital (AOR =3.887; 95% CI;(1.226-12.325).
Conclusion The overall magnitude of women‟s satisfaction with the services was found to be acceptable as compared to different studies. Pregnancy intention, types of health facility, willing to recommend health facility for future health care need and service cost were factors associated with women‟s satisfaction during delivery services. Factors that are attributed to low magnitude of satisfaction are modifiable. Therefore, it is recommended to Organize the service cost as per the standard (free) and strengthening family planning service utilization in the community.
